How Do I Write A Resume For My Teacher To Write A Letter Of Recomendation?

I asked my teacher to write me letters of recomendation and he said for me to make a resume and give it to him. How do I do this? What all should it include?

    You do not say what year you are in at school, so will do a general one that you can adapt!: You can do it typewritten or handwritten – but PLEASE make sure it is legible and your writing is good! Remember this is for a teacher and he knows what you are like, so you have to be honest! Make sure the page is ‘balanced’ – it covers the whole page and things are evenly spaced out! Remember that your teacher is also going to help you, which is why he wants to see your resume – he will make sure the resume looks good and may also be able to suggest things you may not have considered! I know it is unlikely you will have a lot in it yet but you can still make sure it looks good on the page, even if you think the content is not so good!:
    Name
    Address
    Telephone
    Date of Birth
    Name and address of school
    Last exam results
    Any jobs you may have had, including what you had to do (eg babysitting, Saturday job, paper round…)
    Any clubs and activities etc. that you are a member of, inside and outside school
    Any hobbies (but make sure you can talk about them! – the number of people who say ‘reading’ but cannot say what book they read last is amazing!)
    You could also list your best attributes – eg tidyness, promptness, organisation etc.
    Name of referee (s) – I suggest you have 2, one of whom is an old family friend; hopefully your teacher will be the other one
