How To Write A Resume For A Teacher Recommendation?

My homeroom teacher would be really honest about me in a recommendation (even though we’re not on lovely terms), because she’s known me and my habits/mannerisms the longest, but I overheard her asking for a resume from my classmate when he asked for a recommendation.
I’m not sure how to format/begin/write a non-job resume. Help?

    I’m not sure how old you are, but then it seems that you need this for academic purposes so here’s a format that you can use hope this is helpful:
    Name
    Add
    Contact num:
    Email:
    Educational Highlights:
    (means your achievements in school, any achievement of any kind pertaining to academic performance)
    Seminars and Workshops
    (means all the seminars you’ve attended so far or if you had an activity in school)
    Organizations
    (if you have an organization in school list down together with your position, if your not an officer then write member to date)
    Personal Information
    (limit this to age, date of birth, gender, nationality, religion,)
    Educational Background
    (Where you’ve studied up to now)
    References
    (list atleast 3 together with their contact info)
